Goal and benchmarks are pretty good. I just wanted to clarify one thing. The Level of attainment section is designed to be worded in a way that shows the difference between the baseline and the goal. Your numbers don't really match up. Can you take a look at that? Also, I know some districts promote the use of a percentage AND number of opportunities, but from a statistical perspective, how does one accurately track 25% of the time in 2/5 opportunities? If your district allows, I suggest using one or the other, but not both. Progress monitoring will be much more accurate this way.
Lastly, I know you don't necessarily have a student to do this with, but I'd like you to go back into the Eligibility Checklist an write up a brief statement under the documentation section of the questions you answered yes to. If this were a real evaluation, DPI wouldn't approve your checklist with just the boxes checked. Imagine a kid you have worked with in the past and use what you know from memory to create some statements.
